Expectations for applying FMEA in your Sr. Design Project

 

1. As a team, List many (all) potential failure modes

 

2. Select some (at least one for each team member) failure modes from the list that have high severity or probability of occurrence, making sure they represent several types of failures (not just "Machine Design" type)

 

3. As a team, establish a rating scale for Severity and Probability (describe the scale if different from the recommended scale).  For this project you can elect to ignore probability of detection unless it is a major feature of one of the failure modes.

 

4. Using the recommended FMEA forms (or equivalent), evaluate the potential failure modes, and include a clear justification for your ratings.  Each student should complete an FMEA worksheet for at least one component or subassembly.

 

5. As a team, Compare severity, probability and RPN for the failure modes and use the collected info to define specific corrective actions taken to address the highest priority failure mode(s).

 

6. Do the analysis or complete the corrective actions to reduce the risk of failure to an acceptable level.

 

7. Update the FMEA based on the corrective actions.

 

8. Use the FMEA methodology to help organize the "design justification and validation" sections of your design report.
